Question: What are the qualifications for joining Chicago R.E.A.C.H.?
Answer: Applicants must be:
- 55 years of age or better
- Live in the Chicago R.E.A.C.H. service area
- Assessed by the Chicago R.E.A.C.H. Interdisciplinary Team
- Assessed by Illinois Department on Aging and found eligible for Chicago R.E.A.C.H. services.

Answer: Chicago R.E.A.C.H. is a Medical & Social model of care. We provide all acute to long-term care medical services, recreational therapy, social service, and case management. View a complete list of services offered.
Question: Can I keep my own doctor when I enroll with Chicago R.E.A.C.H.?
Answer: When you enroll, the Chicago R.E.A.C.H. network of physicians provide all of your primary and specialty care.
Question: What if I need medical supplies, who provides it?
Answer: Chicago R.E.A.C.H. is responsible for providing all of your necessary medical supplies.
This might include, but is not limited to:
- ambulation aides such as a walker, cane, or wheelchair
- oxygen & supplies
- hospital bed
- bathroom aides such as raised toilet seats or grab bars
Question: Is there any cost to me for this program?
Answer: There is no cost to you if you are Medicaid eligible. If you are not Medicaid eligible, you will pay a cost equal to the Medicaid payment rate. Please contact the Marketing Department for further information.
Question: What will happen to my Medicaid card when I join Chicago R.E.A.C.H.?
Answer: When you enroll with Chicago R.E.A.C.H. you will receive a Chicago R.E.A.C.H. membership card.
Additionally, the Medicaid medical card you receive in the mail every month will state that you are enrolled with Chicago R.E.A.C.H. MCO (managed care organization) and prior authorization will be needed for services.
All services must be provided by the Chicago R.E.A.C.H. network of care providers or authorized prior to receiving services outside of the network. Participants may be fully and personally liable for any unauthorized services.
